چکیده

Delirium is a frequent complication after surgery with important negative outcomes for affected patients and society. However, it still largely unknown why some have predisposition delirium others not. To increase our understanding of the neural substrate postoperative delirium, we studied association between preoperative brain MRI features occurrence major surgery. A group 413 without dementia (Mean 72 years, SD: 5) was included in prospective observational two-center study design. The conducted at Charité Universitätsmedizin (Berlin, Germany) University Medical Center Utrecht (Utrecht, Netherlands). We measured volumes (total brain, gray matter, white matter), matter hyperintensity volume shape, infarcts cerebral perfusion, used logistic regression analysis adjusted age, sex, intracranial volume, center type Postoperative present total 70 (17%). Preoperative cortical increased risk although this did not reach statistical significance (OR (95%CI): 1.63 (0.84–3.18). Furthermore, found trend an more complex shape hyperintensities 0.97 (0.95–1.00)). volumes, perfusion were associated delirium. Our suggests that those may developing

Preoperative cerebrospinal fluid β-Amyloid/Tau ratio and postoperative delirium

OBJECTIVE The neuropathogenesis of postoperative delirium remains unknown. Low cerebrospinal fluid (CSF) βamyloid protein (Aβ) and high CSF Tau levels are associated with Alzheimer's disease. We therefore assessed whether lower preoperative CSF Aβ/Tau ratio was associated with higher incidence and greater severity of postoperative delirium. METHODS One hundred and fifty three participants (71...

Preoperative anxiety management, emergence delirium, and postoperative behavior.

Preoperative anxiolysis is important for children scheduled for surgery. The nature of the anxiety depends on several factors, including age, temperament, past hospitalizations, and socioeconomic and ethnic backgrounds. A panoply of interventions effect anxiolysis, including parental presence, distraction, and premedication, although no single strategy is effective for all ages. Emergence delir...

Preoperative medication use and postoperative delirium: a systematic review

BACKGROUND Medications are frequently reported as both predisposing factors and inducers of delirium. This review evaluated the available evidence and determined the magnitude of risk of postoperative delirium associated with preoperative medication use. METHODS A systematic search in Medline and EMBASE was conducted using MeSH terms and keywords for postoperative delirium and medication. Stu...

[Postoperative delirium].

This article reviews the incidence, pathophysiological hypotheses, and etiology of postoperative delirium, especially in the elderly. Preoperative, intraoperative, and postoperative risk factors for delirium following surgery are discussed. Results of various studies on postoperative delirium appear hardly comparable due to methodological and population differences. Therefore, it is difficult t...

Postoperative delirium.

Mr. A, a 50-year-old English-speaking Korean man, presented to the hospital with right scapular pain. CT of the chest showed two masses, and a CT-guided fine-needle aspiration biopsy study revealed poorly differentiated non-smallcell carcinoma of the lung. MRI of the brain, with and without gadolinium, showed neither a mass lesion nor abnormal enhancement suggestive of metastatic disease.
